Lister House has updated the Online Access for Patients. This update allows patients access to more of their medical records. To gain access to these updates please come into the surgery with photo I.D. to re-register for Online Access.
Parents/Guardians of under 16s will now need to complete an Online Access Form. For children aged 11-15 they will need to be present when access is requested. Parents/Gaurdians of under 16s will need to re-register for Online Services. Photo I.D. for the patient and the parent/guardian is required.
